Mole Removal NYC – Advanced Techniques for the Face, Nose, and Body by Dr. Gary Jayne Rothfeld

Mole Removal NYC: Mastery in Precision and Care
By Dr. Gary Jayne Rothfeld

In this world, where appearances carry weight and carry meaning, the removal of a mole from one’s face, nose, or body is not simply a medical procedure. It is an act of understanding, of carefully navigating the line between science and artistry. Mole removal demands precision, expertise, and a sense of profound respect for the person who walks into my office seeking both restoration and peace of mind. I do not simply see a mole. I see a person. I see their story, their experiences, and the ways in which they have been shaped by time, circumstance, and identity.

Advanced technologies in dermatology have granted us the tools to remove these marks with unparalleled accuracy, whether on the face, the nose, or the body. At 629 Park Avenue, in the heart of New York City, I offer an approach that combines these tools with a deep respect for aesthetics and function. Our methods are not just modern; they are pioneering.

For the face, where every feature carries significance, the approach to mole removal is delicate. Whether you seek the removal of a benign mole or one of medical concern, our team uses state-of-the-art lasers and precise excision techniques. These tools minimize scarring and maximize cosmetic outcomes, ensuring that the skin is left as unblemished as possible.

For moles on the nose or other areas of the body, the technique becomes even more critical. The contours of the nose are unforgiving; each movement must be calculated. Here, the same advanced technology is used, but with added finesse. With fractional CO2 lasers, cryotherapy, and surgical excision, the result is a smooth and even skin surface. Every inch, every contour is respected.

In every case, we make it a point to understand your needs and goals. Every patient is unique, and the approach should reflect that. Whether you require a simple removal or a more complex procedure, rest assured that you are in the hands of a specialist whose mission is to leave you feeling as confident as you are healed.

Mole removal is not a mere technical procedure. It is the art of restoring the self, of giving back what the world may have taken. We see you, not just as a patient, but as a person who deserves the very best care, and the very best result.

At Park Avenue Dermatology and Non-Surgical Rejuvenation Center, we are ready to help you take the next step toward clarity, beauty, and self-assurance.

Abnormal or unsightly moles can generally be excised in a brief and straightforward outpatient procedure performed under local anesthesia by Dr. rience in skin cancer treatment, particularly in the treatment of malignant melanoma. Dr. Rapaport’s experience and expertise in skin cancer treatment makes him an ideal specialist for the identification and treatment of both “normal” non-cancerous moles and “abnormal” pre-cancerous moles. Along with his knowledge of skin cancer treatment, Dr. Rapaport’s aptitude for cosmetic surgery enables him to minimize the potential of an unsightly scar formation during the mole removal.
When Should You Choose to Remove a Mole?
Most moles are benign or non-cancerous. That said, because there is always a possibility that a mole is cancerous, it is best to get medical advice if you notice a mole that suddenly changes in size, shape or color, bleeds, itches, or becomes painful.
Types of Moles and Their Characteristics
Skin Tags
Skin tags are small growths that generally have small and narrow stalks. Skin tags are usually skin-colored, but may also be darker in color, and are usually painless.
Flat Moles
Raised Moles
The Mole Removal Procedure
Mole removal surgery and recovery generally is simple. If the depth of the mole merits an excision, stitches are used for closure and are left in for approximately one week. If the mole is superficial, it may be shaved, and no stitch will be required. In these cases, the mole removal site will form a scab that will fall off within a week. Once the scab comes off, the underling area is usually a pink or reddish color, which may take several weeks to blend in with the surrounding skin. Regardless of the technique used for mole removal, the area should be kept clean and protected from the sun with a high SPF.
A Warning About Shaving Moles
As noted above, it is possible to remove a mole by “shaving” it off. Shaving a mole offers the advantage of not having sutures, but has the clear disadvantage that if the mole extends beneath the surface of the skin, as most do, it will predictably grow back, usually within weeks of being shaved off. It is for this reason that does not recommend shaving off moles. Many patients who go to a dermatologist complaining of moles will end up having the moles shaved, because many dermatologists are not comfortable with performing excisions and fine suture closure. Many dermatologists deal with these moles by performing what is called a “deep shave.” This means that the lesion is essentially “scooped out,” with the shave extending lower than the surface of the skin. While this is more likely than a flat shave to remove the entire mole, the resulting scar is generally quite unfavorable, appearing as an indentation in the skin, much like a chicken pox scar. does not perform deep shaves for the reasons noted above.
How Long Does a Mole Removal Take?
Mole removal surgery generally takes less than half an hour.
Anesthesia
es local anesthetic to minimize discomfort during the mole removal process. Many patients also opt to take an oral medication (such as Valium or Ativan) before the procedure starts to be more relaxed.

Cosmetic removal of moles is one of the most popular cosmetic dermatology procedures today. Although most people refer to any non-acne bump on the skin as a "mole," many different varieties of skin lesions existvariety of techniques to remove moles. The technique depends on the diagnosis and the severity of the condition.

Fortunately, we have a number of safe and non-invasive ways to remove unsightly growths, especially on the face or sensitive areas of the body. Different growths need different procedures, so it's important to discuss the pros and cons of each method with your doctor before you decide to take the plunge. One of the most popular and advanced methods for removal of a non cancerous skin growth in cosmetic sensetive areas of the body is blade-free Radiosurgery.

Blade-free Radiosurgery is a safe and stress-free way to remove benign skin lesions. Radio waves replace a scalpel to make a bloodless, micro-smooth incision. Instead of pressure or cutting, with radiosurgery, high-frequency sound does the job using a programmable wand-like instrument.
With radiosurgery, you have reduced risk of significant scarring because it causes minimal damage to the surrounding skin. Radiosurgery is especially useful for treating the face. For many patients, the injection of a small amount of anesthetic is the only discomfort they feel.

Moles, skin tags, warts, and other raised lesions, as well as sun spots and facial veins can all be treated with radiosurgery. After the procedure, all that remains of the original lesion is a raw, smooth, but non-bleeding area. A scab forms, and within two weeks the scab is replaced with a fresh layer of healthy skin that is the same color as the surrounding tissue.

Why remove a mole?
Most people with facial moles feel a bit self-conscious about them. Removing those lumps isn't about vanity; it's about a boost to confidence and self-esteem. Another reason to remove moles, cysts, warts, and skin tags is that in some cases they can be painful or restrict normal movement.

Health concerns are an even more important consideration. Skin cancer is always a danger with mole-like lesions that are multicolored or irregular in shape. While most simple moles are benign (non-cancerous), it's a good idea to have lesions removed and tested.
Whether your moles make you worry about skin cancer, or just make you frown an make the process of removing moles relatively simple, fast, and pain free.

RADIOFREQUENCY SURGERY POST PROCEDURE INSTRUCTIONS
Please treat all the areas as follows:
1. For the next 7-10 days, wash the areas that were treated at least 1-2 times per day with Baby soap and water. (Sometimes the physician will want you to use a special type of covering for a week. If so, follow those special instructions.)
2. After washing, apply a thick layer of Aquaphor ointment or Vaseline that is over the counter. This will not only be soothing, but will also help the area to heal and minimize chances of skin infection.
3. The area must be kept moist with recommended ointment by applying it 2-3 times per day. Keep treated area covered with sterile gauze/band aid until it heals (about 5-10 days).
4. Never apply gauze or band aid to the wound without recommended ointment since it may cause gauze/ band aid adhesion to the wound and wound’s poor healing.
5. You will note redness and minor swelling around the area where the lesion was removed. This is to be expected. However, streaks of red leading away from the area or progression of redness and/ or swelling and pain or pus like discharge might signal an infection. If you see these changes and their worsening from day to day, please call the doctor right away.
6. Remember that any and all lesions can return. If a particular abnormality was removed and appears to come back or looks changed or different, please return to the physician for a follow-up examination.

What is Radiosurgery?
Radiosurgery is a technology used to treat a variety of soft tissue conditions, from the skin to deeper tissues. We use the latest technology to ensure safe and effective removal of moles, warts and skin lesions. Instead of using a scalpel, radio waves do the cutting.

Radiosurgery technology is somewhat similar to laser or electro-surgery, but it uses a different part of the electromagnetic spectrum than either. Instead of using light waves (like laser), and instead of using high-temperature, low-frequency waves (such as with electro-surgery), radiosurgery uses low-temperature, high-energy waves in the radio bandwidth.

What's the benefit of using radio waves instead of laser or electro-surgery?
The low-temperature waves used by radiosurgery means that tissues are not burned like they are with electro-surgery. Radiosurgery high energy waves penetrate much less deeply into peripheral tissue (0.02 mm) than CO2 laser (0.5 mm), Holmium laser (0.05 mm), or electro surgery (1.0 mm) units. The benefit to patients is that there is substantially less peripheral tissue destruction (by a factor of 25x compared to laser and by a factor of 50x compared to electro surgery). Less peripheral damage to tissues means much greater precision and significantly faster healing times. Simply put, radiosurgery is an extremely precise way to perform surgery, creating much less trauma to surrounding tissues than other technologies. We use the latest technology to ensure safe and effective removal of moles, warts and skin lesions. Radiosurgery is a technique for excising and ablating soft tissue lesions that has established its role as a useful tool in dermatology and other specialties, particularly neurosurgery<

What areas are most suitable to perform Radiosurgery?
Radiosurgery effectively removes outgrowths present on any part of your body:
Face: The technique is particularly useful for facial growths and acne spots as there is no scarring and the healing period is short.
Armpits / Breasts / Neck / Groin: The technique is often used to remove skin tags under the armpits or breasts, around the V-line of neck, and in the groin.
Arms / Legs / Back: It removes brown spots, solar keratoses, and other growths

There are various benefits to this procedure, including:
Removing protruding moles that get in the way of shaving.
Reducing skin irritation that can occur when certain moles rub against clothing or jewelry.
Achieving smoother, clearer skin.
Enhancing appearance and improving self-esteem.
Most importantly, if a mole is suspected to be pre-cancerous early on, it can often be completely removed before it causes a serious health risk.

Who would best represent an Ideal Candidate for Mole Removal?
Those people who can physically and emotionally benefit from having their mole removed would be considered a good candidate for the procedure. However, people must remember that there are limitations to what cosmetic surgery can do. It is meant for improvement, not perfection and is important to have realistic goals and expectations about mole removal.

Mole removal procedure:

The lesion/mole is first prepped with isopropyl alcohol, injected with a small amount of lidocaine 2% (anesthetic), and then painlessly removed using a radio-surgery electrode*. During the procedure, the treated area is cauterized as the lesion is being removed. This means that the treated area is free of bleeding and therefore ready to be prepped with a triple antibiotic ointment and a band-aid immediately afterwards.

After the procedure, patients are asked to keep the treated area clean and free of contaminents until the wound has scabbed over. This post-procedure maintenance is done by simply cleaning the wound with mild soap and water, applying the triple antibiotic ointment (supplied by us), and a band-aid 1-2 times a day for 2 to 4 days. Typically patients see new skin after a week which is usually pink in color. The new skin then gradually starts to match the surrounding skin in days to weeks.

Skin Tag Removal with Radiosurgery
skin tag removal and cost

If you're wondering what a skin tag is, then you probably don't have them. Those who are affected by skin tags, especially large numbers of them, are often a bit self-conscious about wearing skin-bearing clothes. Skin tags are those tiny pearl or seed-like moles that appear to hang off of the skin and look as though they could be easily removed but in fact, are quite firmly attached to the skin's surface. Whereas a mole is connected to the skin by its entire surface, a skin tag is attached only superficially by a tiny point at its base. As we get older, we may notice a growing abundance of skin tags, particularly on the face, neck, chest and arms.

Skin Tags, also known as cutaneous skin tags, fibroepithelial polyps or acrochordons, are usually benign (non-cancerous) skin growths. These growths that are usually small are very common in people older than 30 and are thought to occur from skin rubbing against skin or clothes and shaving. These tags are generally located on the neck, armpits, trunk, body folds, and other areas. They are generally flesh colored (occasionally darker). They stick out of the skin and may have a short narrow stalk connecting to the skin surface. They are usually harmless and painless however they may be irritated from rubbing on clothing or other materials.

uses the radiosurgery method to successfully and permanently remove skin tags, leaving no visible scars.

Warts are caused by HPV (Human Papilloma Virus), which is a DNA virus. More than 100 different types of HPV have been identified. These include Common Warts, Palmar or Plantar Warts, Flat Warts, Cystic Warts, Genital Warts and many other types. HPV is spread by direct or indirect contact. Autoinoculation may also occur, causing local spread of lesions. Multiple modalities are available for the treatment of warts, but most are not uniformly effective.

These include topical agents such as salicylic acid, canthardin, trichloroacetic acid, podophyllin and more. Some physicians have used cryotherapy (freezing with liquid nitrogen), lasers or prescription systemic and topical medications, which have also shown minimal effectiveness.

Remove unsightly or changing moles with minimal scarring using our unique dermatologic, subcutaneous (“under the skin” stitches) closure technique. We remove moles that you don’t like the look of, but more importantly, ones that fail our ABCDEs survey for potential malignancy. (see our Special Topics section of our website on identifying a suspicious-looking mole.)
Professional Laser Mole Removal and Scar Treatment
Suspect moles are removed using a state of the art laser. Our NYC dermatologists then place deep dermal and superficial sutures. Deep sutures bear all of the tension of a wound, and the superficial sutures approximate the epidermal edges for a cosmetically ideal result. Deep sutures are absorbable and do not need to be removed.
On the face, cutaneous sutures may be removed after about 5 days. On the body, they may be removed between 10-14 days. Prolonged placement of superficial sutures can result in regrowth of skin over the sutures, as well as the formation of unsightly permanent track marks, so we take our sutures out as early as possible.
Sutures are available in various thicknesses that are suitable for different body sites and tissues. As might imagine we need thicker, stronger sutures for the body than we do for the face where there’s far less tension.
Our highly skilled team of New York City Dermatologists will choose the safest and smallest suture that allows the wound edges to touch without cutting into the tissue. Larger-than-necessary sutures do not add to wound strength, attract more bacteria, and end up leaving non-ideal scar patterns.
It’s important to take notice of abnormal moles. If you notice ANY changes or simply don’t like the look of your mole, schedule an appointment at our NYC Dermatology Office or our Brooklyn Dermatology Office.

What to Know About Moles

How do you know if you need mole removal? The answer can be a trick one. Many people have numerous moles of varying shapes and sizes on their bodies. Most of the time a mole is just a benign pigmented lesion on the skin, but it is extremely difficult to know if your mole is suspicious or could potentially lead to problems in the future. Having a board-certified dermatologist examine your mole to determine if you need mole removal is imperative. Regardless, it is often ideal for people to have their moles removed. Many of our patients have moles in places that are cosmetically not ideal for them..

Moles and Skin Cancer

There is an increased risk of skin cancer for those who do not remove their moles. A mole can change color, shape or texture, which are all warning signs, and you may not even realize it. Skin cancer is 100% preventable, as long as you are smart about seeing the dermatologist for regular skin cancer screenings. It’s also important to check your skin at home. Know your ABCDEs of skin cancer. The letter “A” is for asymmetry (when the mole differs than the other half), “B” is for border (an irregular border), “C” is for color variation (a change in color), “D” is for diameter (if a mole is larger than 6mm), and “E” is for evolving (any mole that has changed shape, size or color should be evaluated by a dermatologist).

Mole Removal Methods

The board-certified dermatologists at Schweiger Dermatology have received specialized training in mole surgery and mole removal. As soon as it has been determined that your mole needs to be removed, our physicians will talk to you about the various options for mole removal. One of the easiest methods of removing a mole is with a technique called excision, which is “shaving” the mole off with a scalpel either flush to the skin or right below it. Directly after excision, your dermatologist will send the mole to the laboratory to determine if it is precancerous or benign. The area is cauterized to stop the bleeding. Laser mole removal is another method that is used, but since the laser destroys the pigment, this method is not recommended unless the mole has already been determined to be benign. It is important to have the mole sent in for pathology prior to laser removal, to make sure the mole is not cancerous.

The best candidates for Mole Removal Plastic Surgery
Mole removal encompasses the removal of skin tags, raised moles and flat moles. The best candidates for this procedure are patients who are either dissatisfied by how a mole looks cosmetically or are concerned by the appearance of an irregularly shaped, colored, or sized mole. These growths appear as flat or raised clusters of heavily pigmented skin cells on or above the skin's surface.

While most pose no danger, some moles may develop into malignant melanoma. Particularly worrisome moles are those that have mottled colors or irregular edges. If you are concerned or dissatisfied by the appearance of your mole, or if your mole is constantly irritated from rubbing against clothing and jewelry (which can sometimes cause pre-cancerous changes), you may want to consider mole removal surgery.

All Mole Removal Surgeries carry some uncertainty and risk

Like any surgery, mole removal carries some level of uncertainty and risk. Mole removal can enhance your appearance and relieve uncertainty about a worrisome mole. Should the worst occur and the presence of cancerous cells is confirmed by a biopsy, swift and lifesaving treatment can be implemented. No one wants to hear that their mole is cancerous, but early detection and treatment are crucial to a successful recovery.

lth during your initial consultation to determine where your moles are located, how much they interfere with daily activities and carefully select the best technique, either shaving or excision, to remove each individual mole. will explain the risks involved with surgery, other surgical options if available, and a combination of mole removal and scar revision if necessary to minimize any potentially permanent scars.

Preparing for your Mole Removal

Depending on the location, size, and individual characteristics of your mole, surgery may be performed the same day as your initial consultation and will only involve local anesthesia. For larger moles, growths found on sensitive locations, or worrisome moles, patients can schedule a separate appointment to ensure that the entire mole and surrounding potential pre-cancerous cells are excised.

Where your Mole Removal Surgery will be performed

Mole removal may be performed in-office using local anesthesia. It's usually performed on an outpatient basis for convenience and cost containment, and patients may return home immediately following surgery. Patients should anticipate returning for a post-operative visit one week following surgery, so Dr. Antell can assess the progress of your healing and remove stitches if the excision technique is performed.

Types of anesthesia for Mole Removal Surgery

Mole removal surgery utilizes local anesthesia and may also require the use of an IV sedative commonly known as “Twilight,” but the use of IV sedation is rare. If “Twilight” is utilized, patients will feel as though they are falling asleep naturally. You won’t be aware of the surgery or remember the surgery taking place despite being conscious throughout your mole removal. The use of “Twilight” ensures that patients are comfortable throughout the entire experience.

The Mole Removal Surgery

For some of the flat moles that occur later in life, a "shave excision" may be appropriate. This technique of mole removal is slightly less invasive than mole excision. Patients can expect a scab to form in the mole’s absence. This scab will flake off after approximately one week, leaving new, reddish skin. This skin will eventually return to a normal pigment within a few weeks and should be protected from the sun and kept clean during this time period.

In most instances, however, these darker pigmented cells are deep in the mole and shaving alone will still leave a dark spot.  Full thickness excision is the definitive treatment and is the technique that is most often performed. This method will also give the pathologist the best specimen to evaluate the cells for any pre-cancerous changes. Patients can expect to receive a few stitches with this technique and to schedule a follow-up appointment to remove stitches approximately one week following surgery.

After your Mole Removal Surgery

Although patients should expect to schedule a post-operative appointment to ensure the treatment site is healing properly, patients may return home the same day, directly following their mole removal. Whether the mole was suspect for medical reasons or was simply removed for cosmetic enhancement, a sample will typically be sent to a laboratory for testing, ensuring mole cells are not cancerous. Although many moles that are removed for cosmetic purposes are not cancerous, strict precautions are always made to ensure the health and safety of each patient.
